Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in various nursing environments, including hospice care, geriatrics, and medical-surgical settings. As an LPN, you will play a crucial role in providing high-quality patient care, assisting with daily living activities, and ensuring that patients receive the best possible support during their treatment journey.

Responsibilities

Provide direct patient care under the supervision of registered nurses and physicians.
Administer medications and treatments as prescribed by healthcare providers.
Assist with patient assessments and monitor vital signs.
Catheterize patients as needed and provide wound care.
Document patient information accurately using electronic health record systems such as Epic.
Collaborate with healthcare team members to develop and implement individualized care plans.
Educate patients and their families about health conditions, treatments, and preventive measures.
Support patients with daily living activities, ensuring their comfort and safety.
Maintain a clean and organized medical office environment.

Requirements

Valid Licensed Practical Nurse (LPN) license in the state of practice.
Experience in hospice care, geriatrics, or medical-surgical nursing preferred.
Proficiency in using electronic health record systems; familiarity with Epic is a plus.
Strong communication skills and ability to work effectively within a team.
Compassionate demeanor with a focus on patient-centered care.
Ability to perform catheterization and other essential nursing procedures.
Knowledge of medical terminology and patient care standards.
Commitment to maintaining patient confidentiality and adhering to healthcare regulations.

Pay: $26.16 - $36.29 per hour
